    The Homestead is a charming, Assisted Living residence located in Rochester's 19th Ward. We provide four levels of care including assisted living, and three distinct levels of Memory Care for persons with mild cognitive impairment to those experiencing advanced stages of dementia. Respite and enhanced care are also provided.

    MEMORY CARE SERVICES

    Assisted Living /Early Stage Memory Care

    The Elders residing within the “It Takes Two” family need support and assistance to maintain their independence and health while pursuing an active lifestyle. All of these residents need assistance with medication management and health supervision. Some need assistance with showering and personal care. Elders living in our 2nd floor residence dine in our main dining room and attend activities and outings that are offered throughout the Homestead.


    Mid-Stage Memory Care 

    The Elders residing within the “It Takes Two” family need support and assistance to maintain their independence and health while pursuing an active lifestyle. All of these residents need assistance with medication management and health supervision. Some need assistance with showering and personal care. Elders living in our 2nd floor residence dine in our main dining room and attend activities and outings that are offered throughout the Homestead.


    Advanced Stage Memory Care & Enhanced Care

    The Penthouse Sweets family lives in our 4th floor residence. These Elders demonstrate significant memory loss that has compromised their functional abilities. All of these residents need care and assistance in all aspects of daily life. Enhanced Care is also offered for Elders living in the 4th floor residence who require assistance with mobility, management of emotional or behavioral challenges and/or assistance with eating.


    ** Our Memory Care residences are licensed by the Department of Health as Special Needs Assisted Living Residences (SNALR). The 4th floor has an additional license as an Enhanced Care Assisted Living Residence (EALR).

    RESPITE CARE PROGRAM

    Our respite care program at The Homestead provides temporary care – from two (2) weeks to six (6) weeks – and is ideal for Elders who need transitional support after an illness. It also allows caregivers to travel, focus on family issues, or just take a much-needed break. Many respite guests enjoy their stay so much they become permanent assisted living residents.

    ASSISTED LIVING

    Homestead offers assisted living services in our North Hall. The North Hall features living rooms with a fireplace, a large dining hall, library, ice cream parlor and is one hallway down from the beauty salon and fitness center.

    • How do I know if my loved one needs assisted living care?

      If your loved one struggles with ADLs (activities of daily living), experiences safety concerns, or shows signs of isolation, assisted living may be beneficial. A professional assessment by a healthcare provider can help determine the level of care needed.

    • How is medication management handled in assisted living?

      Trained nursing staff assist residents with medication management, ensuring proper dosage and timely administration. Regular assessments are conducted to monitor medication needs.

    • What security measures are in place to ensure residents' safety?

      RPH implements a variety of security measures, such as secure entry systems, emergency response systems, and trained staff to ensure residents' safety.

    • How are meals provided, and are special dietary needs accommodated?

      RPH provides three nutritious meals a day in a communal dining setting. Special dietary needs, such as vegetarian or medically prescribed diets, can be accommodated.

    • Are there social and recreational activities for residents?

      Yes, RPH offers a range of social and recreational activities, including exercise classes, community outings, hobby groups, and entertainment. These activities promote socialization and engagement.

      Yes, as a 24/7 residence family members are welcome to visit RPH at any time. 

    • What happens if a resident's needs change over time?

      RPH assesses residents regularly, and if needs change, care plans are adjusted accordingly. At The Homestead, residents may transition to higher levels of care within the same building. We are structured by floors (2nd, 3rd and 4th floor) which offer different levels of care and creates for an easier transition.

    • What is the process for admission and move-in?

    • How is communication maintained between staff, residents, and families?

      RPH administrators act as the primary point of contact for families. The administrators facilitate regular meetings, regularly calls and emails updates, and utilizes online platforms to share community events, and important information. This ensures that both residents and their families stay well-informed.


      Additionally, the administrator is available to address any specific concerns or questions from residents and their families, fostering open and transparent communication channels.

    • Can residents personalize their living spaces?

      Yes, residents are encouraged to personalize their living spaces with their own furniture, decorations, and belongings to create a home atmosphere.
